The OMG Cares® team helps your doctor by providing extra support to care for you. Nurses, certified diabetic educators, health educators, Case workers and others work closely with your doctors to help you reach your optimal health. There is no charge for services.
How OMG Cares
DISEASE MANAGEMENT
Patients and family members learn how to manage conditions such as congestive heart failure (CHF), chronic obstructive pulmonary disease (COPD) and diabetes by phone.
CASE MANAGEMENT
Case managers recommend community resources and oversee the coordination of care. They bridge communication between the hospital, nursing home, assisted living, key family members and your doctors.
MEDICATION REVIEW
By phone, during home visits or at a clinic, a Doctor/NP/Nurse explains the effective and safe use of medications.
AFTER HOSPITALIZATION CARE
We meet with patients prior to discharge, creating a comfort level with patients and families. Once we receive a call from a hospital notifying us of a patient discharge, our team will visit the resident in their home within 24-48 hours, and we offer coverage on a 24/7 basis.
PALLIATIVE CARE
Appropriate for advanced stages of metastatic cancer/chronic diseases or cognitive impairment, patients and family members receive psychosocial support and symptom management. Palliative Care is not hospice
Care Coaches
If you have a chronic condition (such as diabetes or asthma) we have specially trained medical experts who partner with your doctor. Our Care Coaches help you set and achieve your personal health goals and help you with care planning and understanding your options when you receive a new diagnosis.

CASE MANAGEMENT PROGRAM
Understanding the many aspects of your care is challenging. OMG Cares® offers you access to nurses who will work with you, in conjunction with your personal physician, to help you understand your medications, lifestyle changes that will benefit you, and allow you to take charge of your health care.
HOSPITALIST / NURSE PRACTITIONER’S PROGRAM
OMG has a team of highly skilled Nurse Practitioners, who are specially trained physicians working in many of Baltimore-area hospitals. Available around the clock, OMG’s hospitalists are highly skilled at caring for hospitalized patients in complex situations. They work in conjunction with your personal doctor to ease your transition back to your home or the next level of care.
ANTI-COAGULATION PROGRAM
Some patients with certain conditions require medications to thin the blood. These medications require close monitoring with intermittent blood tests. Our medical team tracks and monitors these tests and works with your personal physician to adjust medications as needed.
HOMEBOUND CARE PROGRAM
In the unfortunate case that you are seriously injured or ill and unable to travel to the doctor’s office, OMG has a program available where a physician can come visit you in your home and help coordinate care with your personal physician.
Care Connection
There are many moving parts in the health care world. The OMG Cares® program can keep all of your doctors connected and up-to-date with your specific care needs.
They will be able to see what drugs you are taking, and whether you’ve been in the hospital. In addition, your OMG doctors and Nurse Practitioner’s can all share information to better coordinate the care that meets your health needs.
This helps eliminate the inconvenience, cost and risk of duplicate tests or services. Your privacy is very important to us, and you control the use of your personal information.
We put important safeguards in place to make sure all of your medical information is safe, but you may opt-out of the sharing of medical information and electronic medical records at any time.
